CoR Interregional Group on Health and Well-being: "Integrated Care in Europe: the way ahead”

“Integrated Care in Europe: the way ahead”   European Committee of the Regions 6 December 2018, 8:00-9:00 In recent years, Europe has faced important demographic changes affecting the ageing population. As a result, increased chronic conditions, multi-morbidity and an ageing population of both the workforce and patients have led to the development of a new integrated care model across Europe. Being able to successfully implement this model is key, as the rising burden of chronic diseases and of the number of people with complex care needs now requires delivery systems that bring together a range of professionals and skills from both the cure (healthcare) and care (long-term and social care) sectors.  Against this backdrop, the next meeting of the European Committee of the Regions’ Interregional Group on Health and Well-being aims to discuss the challenges and the progress made in implementing integrated care models across the EU and give the floor to regional experiences and viewpoints on the matter. The event will involve a presentation by Anne-Marie Yazbeck from the European Commission, who will give an overview on the recently-published deliverables of the study on "Health system performance assessment – Integrated Care Assessment" and what the European Commission can do to support the scaling-up of integrated care initiatives across Europe. Isabella Notarangelo, health economist at the European Hospital and Healthcare Federation (HOPE) will present the ICT4Life Horizon 2020-funded project, which is aimed at advancing Europe’s leadership role in personalised services for integrated care. The meeting will conclude with a contribution by Professor Jonas Christensen from the Malmö University (SE) on AppSam – Professional Support in Dementia Care – a transnational project in the CareSam cross-border research network aimed at implementing a social innovation model.
